Does anyone have or know where I could locate a decent blank format for a Living Will and a Durable Power of Attorney for a 13 year old. I live in Vermont (USA).
***My son is having brain surgery on the 30th and I would like for him to be able to, at the very least, fill out his wishes and have them on file with the doctors before he goes under.

Wow! I'm so sorry to hear about your son. I hope everything will work out for you! I found this website that has a DIY Living Will Kit. It is only $9.95. I will keep looking but i hope this will help!

Wow, I am so sorry to hear that. You might be able to find one at an office supply, or I think that the hospital might have them. I do know you can also purchase them online, there is a CD for the computer that has them on it, and I think it is less than $50.00
