I have had mine done twice, and neither time did it hurt.
I am going to have to say it is about a 4.5-5, compared to all my other piercings. It really is not all that painful when done. Still everyone has a different pain tolerance.
It will bother you for a bit longer than most other piercings just because of the location, mine bothered me for about a three weeks. Nothing really bad though.
If you take care of it though as you are instructed by your piercer you should have no problems with it.
I don't really know if there are any ups or downs, I guess if you really worry about scars you will have one when you take it out. Up side, it looks really cute!
If you really want to know everything about it; how it will be done, after care, and so on, your best bet would be to go talk to a reputable piercing parlor in your town.
Hope this helps.

I just got a monroe piercing 2 weeks ago. I LOVE IT!! It did hurt alot more then the labret piercing though. But it looks so cute. I would say it hurt about an 8 probably. After the second day it didnt hurt at all. It is still a little swollen, and the piercer said it would take up to a month for the swelling to go down all the way so I can put a shorter post in with out it looking like its sinking into my face and hurting.

It doesn't hurt. You just feel it. There's no real explanation. I got it done almost a year ago and it happened really quickly, the guy was amazing. It's doesn't hurt at all, you just feel a prick and it's done. But make sure you eat something before going - this is important!!
The only other thing was it was annoying for the 2 week or 3 week healing period, but after it was fine.

All piercings hurt about the same. They are burning hot when you have it done, then it's over. With all of the adrenaline though it won't hurt as bad as you think. The most important thing is aftercare. Especially with any piercing concerning your mouth. You've got to, got to, got to, make sure you keep them sparkling clean. Your mouth is the dirtiest place on your body, other than your belly button.

Honestly here's what I think...I think it will someday be a real drag to have a hole and scar on your face...just think about it, because you know piercing holes also are dark and would be really hard to cover up, not to mention the cover ups could clog pores and get infected. A face is like forever, so you have to be sure.
