I've always thought it meant to call out someone or to directly aim a comment at someone. It may also mean to TYPE IN CAPS.

SHOUT MEANS THAT YOU TYPE IN ALL CAPS!!!!!!

GENERALLY THIS GOES AGAINST GENERAL NETIQUETTE AND IS OFTEN INTERPRETED AS INCONSIDERATE, OR EVEN OFFENSIVE.
