Passport requirement for underage child?

I am planning a trip this summer outside of the USA and would like to take my 15-year old daughter. After reviewing the passport requirements for an underage child, it states that the applicant and BOTH parents must be present at the passport facility to apply.

My daughter's mother and I have been divorced for 13 years and share joint custody. Although the mother would not object for her daughter to get a passport and leave the country, it's highly unlikely she would accompany us to the passport facility two hours away and make it an all day event.

If there any way I can do this without her participation? Is there a letter I can bring as a substitute?

This loophole will definately put a roadblock in our vacation plans this summer

If one parent appears, you can a notarized statement of consent from the other parent. This is done through form Form DS-3053. You can print the form here: http://travel.state.gov/passport/forms/ds3053/ds3053_846.html Note that your ex-wife needs to fill out and sign the form and have it notarized.
