Many years ago, I saw a car leave the pits and all 4 wheels fell off - not sure how they managed it. As I recall, it was Mark Martin back when he drove the Folgers coffee car, and it was on a road course. Oops.

The Mark Martin event at Fontana comes to mind, but the recent fubar when the crew member chased the wild tire into the grass, causing a caution, is the most recent in my mind.
