Hypoxic heart muscle would have EKG (or more correctly, ECG) changes in the T-wave (flattening or inversion) are classic for cardiac ischemia (but can also be due to other factors). Also, in an acute MI, the ST segment would be elevated "fireman's hat sign", or "tombstone T's". The most valued indicator of cardiac ischemia is not the EKG, but the cardiac isoenzymes (CK-MB, Troponin-I, BNP (B-type natriuretic enzyme)
