Well ... you left that door wide open lol. You didn't specify budget. Its endless what you can spend, but ultimately the best system is the one that sounds best to "your ears".
The best systems will most certainly be designed, purchased and installed by one of the many independent high end audio video dealers. They carry the better lines of equipment speakers, and they generally have many years experience in designing and installing these types of systems.
One thing for certain the better or best systems will use separate components (not a receiver) from manufactures that make the best equipment that they specialize in. There are only a handful of manufactures that to everything well, Goldmund, MBL etc. So generally the better systems will be designed using the best from different manufactures. Some manufactures specialize in speakers, and others electronics, and then the trick is combining the right choices of speakers and electronics that complement one another and work well in the room it will be used in.
I have been designing, selling and installing some of the very best in high end audio video systems for over 40 years, and my job was to determine how my clients like to hear music and sounds, and then replicate that sound in their own home or dedicated listening rooms.
It takes years to understand all the complexity's of designing a truly state of the art audio or video system, so most people should spend most of there time finding the best sales person that listens to there needs and desires in a system, and will take the time to help them create the system of their dreams.
