How to write polite reminder, a beginning ?

'Kindly' is an adverb while 'friendly' is an adjective. Since 'reminder' is a noun, only 'friendly' is a grammatical modifier - so don't use 'kindly reminder'. You could, however, say 'kind reminder', but 'friendly reminder' is more common and sounds better. 'Polite reminder' is also a good alternative.

It is usually phrased friendly reminder. It is more often used on signs than in conversation.
