Hep C count is referring to what they call your "viral load." Viral load is the number of viruses you have per unit of your blood (it's usually per 1mL of blood, I believe). 18 million is fairly high. I'm surprised your doctor gave you this information with no other guidance -- I would recommend making another appointment with your doctor and asking them this very question.
Two new medications for Hep C that are very promising and very effective were just approved by the FDA last year (Olysio (simeprevir) and Sovaldi (sofosbuvir)). Ask your doctor about a treatment plan and take good care of yourself!
