help with Javascript Price Calculator?

Hi i wrote a code for a price calculator where you are supposed to enter you price state residence etc on a pop-up prompt, but i dont know how to get the pop up to work or how to display my final sum

. replace all "clubmembership" to "clubMembership"
. replace all "toUppderCase ()" to "toUpperCase()" no whitespace in between
