I don't want to endanger our relationship or hurt her feelings but she has really bad breath. I'm thinking it may be tooth and gum problems. It's really hard to face her with out breathing through my mouth only. What would your do?

I would sit her down and really gently and lovingly tell her how much I love kissing her and how the breath issue is making it difficult, and ask her if she thinks she needs to see a dentist.

This could denote poor oral health as you suggest, or something else. It's called halitosis and can normally be treated easily enough.
You could buy her some mints, and she might take the hint. Best to tell her straight I would think - because you are concerned for her health.
