Revelation is all about Jesus' judgment and destruction of ancient Rome, the capital of Satans earthly kingdom. Most its content transpired during the dark/middle ages of history. The bubonic plague that GOD sent as one of her 21 curses wiped out a third of the population of that time.

If you read through the book of Revelation (using the idea that these are future events that will fulfilled at the very end of time) you will find there is more than one plague that takes out "1/3" of the population. Some happen before the sections that talk about the anti-christ. Others happen afterwards.
