With all of the covid-19 cases how do you know that they are real? how do you know that the news media isn't fabricating the numbers ? How do you know it's not a case of misdiagnosis. How do you know for example a car crash wasn't listed as a covid-19 death for money.? If you know anybody that's been diagnosed with covid-19 how do you know they weren't misdiagnosed and not suffering from breathing problems due to some other unknown issue?

Because we know how many people die in a normal year, and how many died in 2020.
And you're right that there is misdiagnosis -- but in the opposite direction of how you think. The official number of non-COVID deaths was about 100,000 more than in a normal year, even though there were really fewer non-COVID deaths, because the measures taken against COVID prevent many flu deaths.
The real number of COVID deaths has to be at least 100,000 more than the officially report number.
We know that they didn't all die from non-COVID because there are too many deaths -- we never have that many without a pandemic.
