First off it is Dilaudid aka Hydromoriphone. It is a narcotic. It is not a vascular dilator, but can have a similar effect. It acts on the central nervous system causing a systemic drop in blood pressure that occures due to sedation. And typically it will not raise bp in a person with a normal bp. Now pain that is still not controlled even after taking the dilaudid is more a cause for a rise in bp after taking the med.

Dilaudid intravenously is the strongest of the strong

Intravenous Dilaudid at 4mg should take care of ANY pain.
