Scenario one: Lucy Jennings is a teacher. Explain how the use of an OMR system could be an advantage to both her and her students during an examination involving multi-choice questions.
Scenario two: As a librarian Dipesh Mistry wants to improve the present card index system of lending books. If each book is given a bar code and each borrower has a bar code on their ticket can you describe how this will help him?

Scenario 1: The use of an OMR could help Lucy speed up the process of grading tests. By reducing the time spent for Lucy to grade tests, Lucy can teach her students better by spending the extra time into class preparation. In addition, students could quickly get their tests back, allowing "learning from mistakes" while the test is still fresh in mind.
Scenario 2: The librarian could track who borrowed which books. Since each book is given a bar code, there are no ambiguity concerning variation in titles, duplicates or long titles. This would allow the librarian to track the borrowed books more efficiently.

I don't think it's possible because surely doctors ought to be able to tell if a patient is alive or dead. However, this is a true story. At a hospital in the UK about 11 years ago, they took a "dead" woman to the morgue. Later she woke up and walked out. So clearly it could happen, but it's not likely.

two: If the library system is computerized, there will be much more data available and it will be much easier to find. All kinds of things will be available with an online search: who has book x out now, which books are most used, which books does Sam have out now, which books are overdue, etc.
