My stool hurts when it's coming out! (and more issues)?

I know this is a bit odd and embarrassing, but here it is. I'm on hypothyroid medicine and stool softeners, but I still only go #2 every other day or so. And when I do go, the first "turd" is amazingly huge, causing me great discomfort. The strange thing is is once that first "turd" is out, the rest comes out normally. Whenever I go to the bathroom at home, it clogs. It takes me chopping it in half and plunging to get it down. The only toilets that seem to be able to handle my stool is the public restroom toilets, as they have much better suction. I'm a 23 year old guy and have been dealing with this for most of my life. Help!

Update:

My diet consists primarily of Subway sandwiches (with lots of veggies), frozen meals (Lean Cuisine, Smart Ones, etc.), yogurt and cheese, Cheerios (my nightly snack), and some fruits.

    It's almost certainly not your diet, but your hypothyroid condition that is causing your huge painful turds. Folks with hyper-thyroid tend to have very loose and frequent stools. It's just the opposite for those with a hypo-thyroid condition. You may need to have your medication adjusted. Failing that, try Metamucil every day just before your largest meal of the day. Metamucil can actually help people with loose stools as well as constipation by regulating how much water is allowed to stay in the stool. In your case, Metamucil would allow more water to stay in that first turd, making it easier to get out and flush down. Metamucil is just about as natural a treatment you can find. If you try it, you should see results within a week.

    PS: Try to imagine that for a woman in labor, pushing a kid out is at least 100 times worse than pushing out that first turd!
