what is the title equal to in USA that is called a social sundheds hjælper in Denmark? ( have graduated as a SSH in Denmark but dont know what jobs i can get in the states with this work experience)

If I was you, I would contact a place in the US where they work with disabled and handicapped and describe your education and your qualifications and simply ask them how best to get a job in the US in that line of work. Maybe they would know best what to do.
I can't think of anything more simple to do - It's what I would do if I was in your situation!

EDIT: Det gir mening!! Min søster har ikke uddannelsen men har arb. med voksne handicappede - Det var en form for bofællesskab. Jeg ved ikke om det var privatejet, men måske hvis du går om county eller state ejede institutioner.

Ok, it seems like they don't really have anything matching SOSU. but they have different healt visitor/ healthcare educations over there that might look like it. It sounds like the educations are managed by the places where you also can work, like the institutions, etc.
The people in the management position usually have a BS or BA in that field, so if you have a degree from "social og sundhedshjælper-uddannelsen" it might not be difficult for you to find work. I would make sure, though that your school can give you translated exam-papers. I think they should have a set of international forms that explains in english your competence and education and so on.
