The 25th has never been used to remove a president.
Section 1 has been used once, when Nixon resigned and Ford became president.
Section 2 has been used twice; the first time when Agnew resigned and Nixon nominated Ford as VP, and the second time when Ford became president and nominated Rockefeller as VP.
Sections 3 and 4, which remove a president from power, have never been used in the 40-some-odd years that the amendment has been in effect.

When Richard M. Nixon resigned the presidency, he was on the verge of being impeached, convicted and removed. He left voluntarily and Gerald Ford, then VP, became the President.

That amendment doesn't and can't remove a President from office. It details what is to happen if/when a President leaves office before the end of their term.

Yes, Nixon resigned, Ford became President...(Also, every President that died in office...from Garfield to JFK)

Yes, a few times..in the case of Lincoln, Kennedy's death the VP takes over. Rest assured Obama's going to be fine and great :).
