Old Disney movie about finding someone's heart?

Has anyone ever seen that old disney movie, well i think its disney.. i don't remember exactly at it was called but the title said something about finding someones heart.

it had to do with this lil girl who was close to her brother, but he had died and his heart was given to someone.

in the movie it shows flashbacks of her and her brother . i think there was this one flashback where she was looking for her brother because it was her birthday of something and then she finds her brother in a barn or something with his girlfriend, but they were actually wrapping her gift or something.

but she goes out and tries to find the person who received her brother's heart. and i think he turns out to be a black child.

i dont really remember much but i really liked the movie. so if anyone could help me that would be great . thankss!
