Why does the tape come out of my audio cassette tape sometimes while it is playing?

Is it the fault of the tape player, or the fault of the tape itself? Is there anything I can do to stop this happening?

Update:

I mean, the actual strip of tape comes out in a loop, out of the cassette tape.

    Its the player's fault. I think the gizmo that the tape slides across has become magnetized and is holding onto the tape. You know, you -could- join the 21st Century and get a CD player instead! I know its tough to give up cassettes but CDs even sound a LOT better with no hiss in the background like cassettes have....
