You can use The Sims 2 Bodyshop which is included in the Sims 2 games. You can create custom sims, save them, and export them to the Sims 2 Exchange.

you need to use body shop. to find it open your task bar
all propgrams- ea games-> sims 2 (or whatever expansionpack you've most recently installed)->body shop
you can just drag it onto your desktop
