A short, simple note is all that is required. Express that this is only for a short time and ask your daughter's teacher if there is any work that she can take with her to do while she is gone, or alternatively, work that you can collect and have your daughter complete when she returns.

(Headmistresses' name) I wish that my daughter, (<name goes here>), in class 3a could leave school early on Friday, October 26th. She is unable to do the cross country run that afternoon due to an operation that she has recently had and she is still experiencing discomfort. Also she will not be able to attend the dance that evening due to plans that have been set up prior to this event. Thank you for your time and understanding. Sincerely (<Parent's name goes here>) (<Your name goes here>)

Just state the reason clearly, im sure they will understand, its not like she will miss a whole month
