How can I have an insurance plan "established under your business" if I am Self-Employed?

. . . this is a condition for line 29 of the 1040 per the instructions. Do I have to have a business with a different name, like the "Jerry Lee Lock and Smith Company" with the plan in that name?

    For purposes of the self employed health insurance deduction, the insurance policy can be under the name of the sole proprietor.
