All you need is your Canadian Birth Certificate and a letter of consent by your parents.

I quote the website of Canada Border Services Agency:

Canadian citizens aged 15 years or under are only required to present proof of Canadian citizenship, such as an original or a photocopy of a birth certificate, or an original citizenship card, when entering the United States by land or water.

    http://cbsa-asfc.gc.ca/whti-ivho/child-enfant-eng.... ..

And the information of the US Department of Homeland Security:

Beginning June 1, 2009, U.S. and Canadian citizen children under age 16 arriving by land or sea from contiguous territory may also present an original or copy of his or her birth certificate, a Consular Report of Birth Abroad, a Naturalization Certificate, or a Canadian Citizenship Card.
