Anchor bolt spinning in concrete. Hammered bolt down with nut attached but spins. nut is stuck pretty good.?
Any tricks for holding threaded bolt without damaging threads? tried thick folded cardboard couldn't hold.

Try pulling the bolt back up as that should pull the anchor into the sleeve.

When you pull on anchor bolt they expand. The hole does not allow them to expand.
Start again with new bolt. This time expand the bolt slightly so that it is not easy to put in the hole. This friction will keep it from spinning

use a pry bar to pull up on the anchor while trying to tighten the nut. thread is probably damaged. you can also use the tip of the hammer drill put it on chisel mode only and put it on the tip of the bolt ( red heads have an indent) use the hammer drill to hammer in the anchor. if you have time you can try and glue the sleeve in place.

Sounds like your hole is too big, about the only way to fix it will be to pull the anchor out and drill a new hole of the proper size. If you had the nut all the way up to the top of the threads, chances are that you have ruined the threads beyond repair when you drove it in. The nut should be screwed down at least a half inch before you start pounding on the anchor to drive it in. Most likely you will have to replace the anchor and put it in a new hole of the proper size.
