What Sharingan does Sasuke get when?
I know his Sharingan is awakened the first time when Itachi kills their family. At that point, it has two circles.
Against Haku, it still has two. When does he get the third? And when does he awaken his Mangekyo Sharingan? And does it have other forms?

Sasuke got the third "Tomoe" (Remember it's the shaped like comma in his eyes. They are therefore called Tomoe.) midway his battle against Naruto at the Valley of the End, it reached completion to have three tomoe in each eye.
Sasuke awakened his Mangekyō Sharingan after the death of his older brother, Itachi. It was after their fight at the Abandoned Uchiha Stronghold.
And then he got his Eternal Mangekyō Sharingan when Tobi transplant Itachi's eyes to Sasuke in order to escape himself from being blind so he can fight Naruto in his full power.

His sharingan activates its third-stage when fighting Naruto at the Final Valley. He awakens his Mangekyo Sharingan after killing Itachi. He upgrades his Mangekyo Sharingan to his Eternal Mangekyo Sharingan by implanting Itachi's eyes into himself after fighting Danzo at the end of the Kage Summit Arc. He first uses the Eternal Mangekyo Sharingan during the Fourth Shinobi World War arc.
