Looking for an old dos first-person RPG game?

I'm looking for an old first-person dos RPG game that I had. I don't remember much about it except that the box had a knight on the front holding a sword. It all took place in one town. It came with a printed map of the town that you could mark where things were.

There were guilds you could join. An extensive dungeon area under the town. Your character could get lycanthropy.

I remember you were trying to find a relative, uncle or nephew maybe...

I've done extensive searching on Google and went through the entire Wikipedia list of role-playing video games and haven't been able to track it down :-(
