    The County Clerk will send you in the right direction. Most major cities would have the documents online. In Los Angeles, CA the form would be a DV110 found on www.lasuperiorcourt.org. When filing for a personal protective order/Temproray/Emergency Order. You will have to list reasons why you want this order. It will be beneficial to have copies of police report numbers ready when you file this application.

    In order to get a permanent one you will have to appear in court. This will be a very stressful event since you will face whoever you want this order on. Make sure you have witnesses, photos, copies of police reports etc. Remember to list all your local agencies as enforcing agencies on the form and keep nine copies with you at all times. Three at the house, in the car and on your person. That way responding units can get a copy right away. If your being stalked too that is usually a crime too depending on your state. File a report on that too, there are alot of resources available to you look in the phone book and see the clerk at the court house for victim hotlines and outreach centers. Make sure you text your family and friends everywhere you go this helps with a timeline and as proof in court too incase anything else happens. ***good luck***
