Credit Card Statement?

I need to send in some glasses to get them repaired and, because I lost the reciept, they said I could use a credit card statement. What exactly do they need to know on that and what can I cross out? Like the account number?

    DO NOT! REPEAT DO NOT send them your statement. Why would they need your statement to repair your glasses?

    Are you saying you are returning glasses to the party you purchased them from to have them repaired. If so the original bill of sale would prove you purchased them from them. Is that what you lost? Even so they should have a record of having sold you the glasses. DO NOT SEND THE STATEMENT!~

    I would black out everything except your name, address and the original charge for the glasses
