Is there an online checklist that will assist me in following their wishes if they should end up in the hospital?

It is actually on the sheet for Power of Attorney for Health Care. You should receive a copy of that to be the proxy, as should their doctor's office.

You can have them fill out an Advanced Directive which expresses what they want done if they cannot make their own health care decisions, and you can get a Health Care power of attorney as the other poster suggested.
