    x = rcos(theta)

    y = rsin(theta)

    z = z

    z = 3

    x^2 + y^2 = r^2, so r^2 = 25 + 9 = 34, so r = sqrt(34)

    theta = arctan(y/x) = arctan(3/5)

    So, you have your r, theta, and z. Hope this helps.
