My guess is probably MORE. I was a volunteer fireman way back when in a small town and we did not have any insurance. IF you were injured on the job they would file for workman's comp to offset some of the out of pocket expenses you incurred going to your own doctor. I suppose if your small town likewise does not insure its fire or police then yes, ACA is going to cost them dearly.

Accident insurance and health insurance are not the same unless comprehensive. Injuries are accidents not illness unless resultant from injury. There is also liability to consider in such cases. It is not the same.
