When would an autopsy be performed?
Friend of the family died back in September and his family was waiting on the death certificate cause nobody would tell them how it happened. They found out today that an autopsy was performed that is why its taking so long to get it. I have known other people that age that die and it wasn't done. What would give them cause?
No details just a letter from a lawyer saying he had died
I guess we can rule out accident or sickness they would have figured it out right away
toxicology like drugs I guess that is a possibility

It depends on the circumstance of the death and the policies of the jurisdiction. Some places have fairly strict rules on when an autopsy is automatically performed.
Even with an autopsy, 2-3 months is odd, unless they are waiting for some toxicology reports. They are best to contact the coroner, who will have the file.

They are preformed when there is not a doctor that can provide a cause of death. For example, if a person is not undergoing medical treatment and is found dead at home an autopsy would be preformed.
