If you are worried about regular latex condom strength, try the polyurethane Avanti series from Durex. It's much stronger than latex, although you will notice a difference in the feel.
There are too many reasons to list for condom failure, but IMO the top 3 are :
1) Improperly putting it on the penis (overstretching it, snagging it with a fingernail while unrolling, not unrolling it to the base of the penile shaft, etc.)
2) Too little lubrication. She'll appreciate more lube too :)
3) Using THE WRONG KIND of lubrication...like oil/vaseline based lubes on a latext condom. Use water based only, unless you are using the polyurethane. But oil based lubricants have been associated with higher incidence of vaginal bacterial infections, so you should try to stay away from them.

I only ever used a Durex condom once, and it broke...
