I worked with many moms and kittens for a rescue group. I would suggest that you get a tiny weight scale the kind used to weigh small amounts less than a pound. They usually are less than $10.00
Then weigh each kitten daily. This gives you a much better idea that they are getting the milk that they need. I would rather that the runt not get the supplement and she drinks from her mom rather than an supplement as there are very important nutrients in the moms milk that the runt will not get if she does not drink from the mom. The mother's milk contains antibodies that are much needed especially by the runt. It is very unusual for a kitten to not get enough milk. The mom's milk supply is good if she is supporting the others. If the runt does not eat, it may have unknown problems, like a birth defect. I believe your mom is right and that the runt is feeding when you don't see her doing so.
The weight will tell the story to be sure. Good Luck! For the sake of so many unwanted and unhomable kittens everywhere please ensure that kitty mom is spayed as soon as possible and the kittens too. Thanks. Oh, one more thing and that is that if you give a supplement it should be kitten milk only, not cow's milk or something else as this could make the kitten very ill and be a cause for it not to drink and it could die.
