Confused about diagnosis of gout with high end of normal range uric acid range.?

Update:

My doctor says I have gout. I have some doubt (didn't rhyme on purpose). The first joint of my right big toe has gotten inflamed twice. High uric acid in the blood is supposed to be the cause. I got it tested and I can't remember the exact number right now but it was on the high end of normal for a man my age. Also, I have pretty obvious toe deformity; hallus varus--how does my doctor know I don't just have a bunion? My doctor couldn't clearly explain how I could have gout with normal uric acid.

The diagnosis of gout cannot be excluded on the basis of normal serum uric acid level. You can look up a study done by JA Logan in 1997 that suggests during acute gout, uric acid level falls. Your doctor probably said you have gout because the most common presentation is sudden onset of pain and swelling of the great toe. However, the principal procedure to diagnosis gout is by doing a joint aspiration to check for what's called Monosodium Urate (MSU) crystals.
