Is it worth the cost?
My child's mother is never there with my son, he is being raised by her parents. She has been seen being very abusive and angry with him; cussing, smoking weed and cigarettes with him in the room. If a guardian ad litem shows up a few times randomly I am going to file a motion to appoint one. If it's only one scheduled visit, its not gonna be worth it because she is just going to stage herself there in a good environment.

A Guardian Ad Litem represents minors in court actions only. What you want is a professional guardian. Just Google "guardian" in your city.
