Would this be legal?
Opening a zoo where everyone who enters signs a form agreeing that they will be shot if they intentionally enter or accidentally fall into an animals enclosure to ensure that the safety of the animals is the number one priority?
If someone doesn't want to accept these terms, they simply won't be allowed in the zoo. For children, their parents will have to sign for them.

Yes, it would be perfectly legal: provided you lived in a and with no law.
You can't shoot people. I don't think I can say it any clearer than that. It is illegal to shoot people, even if you are an a$$hole who is mad an undomesticated wild animal was killed to protect a human toddler.

no matter how wel formed your form, there is no way that you can kill them without consequences. Remeber eutanasia is still illegal even if the patient clearly states that they want to die because they are in unbearable pain? yeah it wouldn't work. Maybe if you have them sign that they enter on their own risk and you have no blame if something were to happen, then you could just let your animals kill the person. In theory that is I guess, you can still be sued if you see someone in need and not help them. Wouldn't work I guess
