I think the expression is said twice in the film, once by Forrest and once by his Mom. His Mom says "is" and he says "was". That would explain why it says "is" on the VHS or DVD box even though Forrest says "was", it's not technically wrong. And even if the scene with his mom didn't exist, it would still make sense to change it for the box because otherwise they'd have to include the "mama always said" part because "was" would not be grammatically correct without it. However, there is a YouTube video where Forrest says "is", but it's from a completely different angle and I'm pretty sure it's an alternate take thats from like a speical feature or something but isn't actually in any version of the film.

He was quoting his mother, but her quote wasn't speaking in the past tense. He quoted it as she said it, which was: "Life is like a box of chocolates, you never know what you're gonna get."

The real quote is "Life was like a box of chocolates" because the introductory clause is "My momma always said", past tense said. It's curiously grammatically correct, using the past imperfect.
Had he said "My momma always SAYS", then it would be: "Life IS like a box of chocolates." But his mother is dead by this time in the story, so it's past tense.
I always thought that was wrong. For example.

I think they only used the expression once, if I remember correctly. And if you think about it... the expression has to be... Life IS ... not Life WAS. He IS Still living. If he stated Life WAS... he would be dead, right?
