No, a denial would not necessarily be right away. It could be at any step in the process, when they find a reason to deny you. It could take 3 - 4 months. Be patient.

It takes as long as it takes. However, I'd check with the bank if you hadn't heard anything.

You should have heard in a couple days. Approval would be subject to a satisfactory appraisal.

Sometime they have to do appraisal and check on comparable in the area, so it could take some time.
