Don't tell her the truth. She has enough problems to deal with without discovering that her boyfriend is so shallow. Gradually turn your relationship into a platonic friendly one. Stop having sex with her.

If it bothers you, let her go. She needs someone who loves her for her, and not the appearance of her skin. She's been through quite enough pain without you adding to it.
