Is it possible for a hospital or urgent care medical bill to never be sent to collections if youre unisured/unemployed ?

It's possible... just not very likely. Most facilities make every effort to collect payment for at least a year. But if it's a non-profit hospital, the tax regulations force them to write off a certain amount of debt annually to retain that non-profit status.

You can always talk to the billing dept.they generally know of a state or federal program that can help, many times if you're reasonable and polite they can reduce the payment. Bring your tax return and any proof of income with you.

No. They will find you and make sure you get the bill, even if it takes years.
