You can tell the surgeon to do whatever you want.
Your hair, your decision.
If the surgeon disagrees, then tell him/her/ or trans/ lezzer etc... to go to hell.

It depends on the type of surgery. Doctors remove whatever is needed based on what type and size on incision is required. Hair can cause infection, so as a patient, your choices are to allow the surgeon to shave whatever they feel is necessary or to not have the surgery. A surgeon will not compromise their standards or change their procedures because you don't want to lose some hair.

No. They just shave the necessary area and surrounding area. Can you refuse? No you can't. It's for hygiene purposes. Your hair is always dirty and full of bacteria. The last thing they want is that bacteria getting on your brain because some hair fell into direct contact with your brain.
Just suck it up buttercup and deal with it. Or don't get your brain surgery.
