Should I get paper sleeves with my lathe cuts?

It's an extra 25 dollars.

Will it really do all that much to protect the disc or just don't bother?

It'll have paper inserts anyway (cover & back), so what's really the difference?

I don't even think any of the lathes I own have paper sleeves...

If you're referring to vinyl recordings, inner sleeves greatly reduce contamination by the pulp that album jackets shed.  Avoid uncoated paper sleeves that scuff the vinyl; the insides should look glossy.
