How do search engines categorize images?

I assume there is some sort of artificial intelligence that does this. Is this based upon text descriptions of images, or on the images themselves? I find it annoying that searches for 'big boobs' turn up so many damn Futanari images and even images of men with erections. Is there some kind of conspiracy to plant these images in such searches?

Furthermore, is there some kind of filter to toss out images in my search that don't belong there? Like a checkbox to delete?
