I have two suggestions you could either drive your boss home and when you get tow your boss's house say hey look its my old boss's house or you could... RUN UP TO HIM PUNCH HIM IN THE FACE AND KICK HIM SEVERAL TIMES THEN YOU TELL PEOPLE HE ASSAULTED YOU AND IT WAS SELF DEFENSE SO U SUE HIM AND YOU GET ENOUGH MONEY FROM THE LAWSUIT THAT YOU NEVER HAVE TO WORK AGAIN.
IT WORKED FOR MY 53rd Wife, but this lead to her divorcing me because she now had tons of money and didn't care for me anymore

Put in your two week notice but make sure you apply somewhere else before you do that. So that way you don't go broke in between times. It's not fun not having money.

Firstly, make sure that you have other employment to go to.
Then, politely tell your boss that you have appreciated working for him, but have another job to move on to

You get another job, turn in your notice, say thank you to the current boss and job, and move on. You don't have to be concerned about being a jerk--you've simply found another opportunity.

You give your two week notice, in writing, saying 'thank you for the opportunity I've learned so much" and you move on. You don't make it personal if you want to keep any sort of reference.

Tell him you are looking for more opportunities than seems available where you work now. Thank him for everything, because you never know when you might need his help.
