What would cause testicles to heat ?

Update:

While in the hospital twice. My testes became so hot I had to put on ice packs. Both times with heart issues.

    --  Extremely tight underpants especially "Y-briefs" plus extremely tight pants/jeans.

    --  It is preferable to wear loose boxers along with pants and jeans that are loose at the crutch.
