Was there ever a movie or TV series based on the book entitled "1984"?

Yes. There were two movie adaptations made. The first was made in 1956 and it was adequate. The second was made in -appropriately enough - 1984. The 1984 version starred John Hurt and Richard Burton. Both were nominated for Academy Awards and Richard Burton won. I think John Hurt was robbed. He allegedly lost 30+ pounds to play Winston Smith in the horrific Ministry of Love scenes. It is a very difficult movie to watch but it is essential that everyone see it and strive to prevent the world of 1984 from happening.

In 1985 Terry Gilliam released a dystopian future movie called Brazil that was loosely based on 1984. The visuals are stunning, the story bleak.
