Tritium is created by several different nuclear processes.  Some are decay processes from heavier elements, some are from neutron capture by lighter hydrogen isotopes.

Any or all of these processes could have created the tritium found on the moon.  Some in deep space before the moon formed, some inside stars, some on the moon's surface itself.

Given its half life, most of the tritium found on the moon today would have been formed in situ - generally by the same process that makes it in earth's upper atmosphere.  Cosmic ray bombardment of other elements.
