Well I'm not getting married or anything

I was an immigrant, so when I first came here and filled out my paperwork, my first name and middle name got switched up, so I have a few variations for my name. For a while, I went with what was on the original paperwork, but now I just want to change my name to the way how I want it to be. Can I do that easily or would they ask for the legal paperwork?

What I did before was I added an authorized user for my own self and put my name the way I want it to be on it.

But now I'm just wondering if I can just call them up and ask them to change my name for me.

If I have to fill out the W-9 form for work purposes, does the address have to match the exact address in the IRS tax return record, or would matched name and SSN be enough? The reason is because I still file my tax return using my parents' address, but I often travel out of state for work and often come back home here and there. Thank you.
